Every spring, in many families, the same hunt begins: pharmacy receipts in a box, dentist invoices in email, the private visit receipt who knows where. In several countries some health expenses can reduce your tax bill, but only if you have the paperwork. And rebuilding a year of them in one weekend means losing some.
This article is about organisation. Tax rules — what's deductible, how much, with what payment and documentation requirements — vary by country and change over time: for your case, rely on official guidance or an accountant. The examples below refer to Italy.
Why it's worth keeping them in order
In Italy many health expenses for yourself and dependants can be partly deducted in your tax return, above a threshold. Some of the data is pre-filled, but not always all of it, and many expenses require documents and, in several cases, traceable payment. Having receipts and invoices in order lets you check the pre-filled return and add what's missing.
Tax aside, knowing how much the family spends on health is useful in itself.
1. Record the expense when it happens
The rule that ends the spring hunt: every health expense is recorded on the day it's paid, with a photo of the receipt. Thirty seconds at the pharmacy or leaving the surgery.

2. Receipts, safe and findable
Pharmacy receipts, dentist invoices, private visit receipts, the glasses invoice: photographed or as PDFs, they go in the family documents, in a folder per year. Thermal receipts fade within months: the photo taken straight away is often the only readable copy by spring.

3. Who it was for, and how it was paid
Two details that matter when preparing a tax return:
- which family member the expense was for: each child, each adult
- how it was paid: card, bank transfer or cash, because for many expenses traceable payment can be a requirement
Note them in the expense entry, and the year-end summary will already be split.
4. The year-end summary
In January, filter the previous year's expenses by the health category: the total, split by person, with receipts alongside. Expenses can be exported to a spreadsheet file for your accountant. What used to take a weekend becomes half an hour.
5. In separated families
Children's medical expenses are often among the extra costs split between parents under the separation agreement. Recording them with who paid and the receipt attached solves two problems at once: the split between parents and the documentation for tax. More in children's expenses between separated parents.
In short
Every health expense recorded the same day with a photo of the receipt, visits and tests that become expenses on their own, receipts in a folder per year, who it was for and how it was paid, and in January an exportable summary. Tax returns stay a chore. But without the receipt hunt.
